This image shows the Create Application dialog box. In this form, you enter the following:

Application 
* Application name
* Subscription type (Monthly or Hourly)
* Application archive name (Options are: Sample Application, Storage Path, Upload Application Archive)
* Notes about the application.

Instance
Instances: 1 (Default)
Memory(GB): 1 (Default)
Version: (Java SE 7 or Java SE 8)

The Cancel and Create buttons are in the lower-right corner.
