The Application section contains the following fields and buttons in the order listed:

Name: SimpleService is typed in the text field.
Subscription: Two options are available (Monthly and Hourly). Monthly is selected.
Application Archive: Three options are available (Sample Application, Storage path, and Upload application archive). Upload application archive is selected).
Browse button: Click the button to locate the archive file for an application. 
Notes: This field contains placeholder text (Enter a description for this application).

The Instance section contains the following fields and values:

Instances: 1
Memory (GB): 1
Version: Java SE 8
CacheCluster: There is a drop-down list.

The Cancel and Create buttons are in the lower-right corner.
