This image shows the Initialize Data from Backup section of the Create Service wizard after Yes has been selected from  Create Instance from Existing Backup. Additional, required fields added after Initialize Instance from Existing Backup is set to Yes include Backup File Cloud Storage Path, Username, and Password.
