This illustration shows the Rules page in the Web Form Designer.

In the top left is the rule name and to the right is the Delete and Edit rule buttons. 

Below that is the rule description, "Determines if the Customer Name field is visible." 

Below the rule description are the following free-form text fields: Name, Description, and Rule. There are values in each field.
