Midland Health, a not-for-profit healthcare system in west Texas, provides primary, specialty, and advanced services, including palliative care to support people living with serious illnesses. To enhance palliative care and address patients’ holistic needs, Midland Health implemented Oracle Health Palliative Care, drawn to its integrated approach, automated patient identification, and real-time alerts for clinicians involved in this care. The solution helps the organization standardize documentation and supports interdisciplinary care. It provides early patient identification in the emergency department or during inpatient admission using Oracle Health’s Palliative Care algorithm, and it helps assess symptoms through industry-standard tools such as the Palliative Care Performance Scale (PPS), Edmonton Symptom Assessment System (ESAS), and Portable Medical Orders and Medical Orders for Life Sustaining Treatment (POLST/MOLST). In just six months (November 2024 to May 2025), the implementation achieved remarkable outcomes. Surrogate Decision Maker documentation increased to 23%, and Goals of Care documentation rose to 31%, significantly surpassing Midland’s 5% target in 180 days.1 These results reflect improved clinician engagement, enhanced care coordination, and stronger alignment with patients, ultimately elevating the quality of life for eligible patients., ultimately elevating the quality of life for eligible patients.
