St. Joseph’s Health is committed to delivering high-quality, patient-centered care to underserved populations in northern New Jersey. With care teams in multiple locations, including hospitals, physician practices, urgent care facilities, and outpatient centers, St. Joseph’s Health struggled to access and reconcile patient data scattered across disparate EHR and payer systems. Manual processes and fragmented reporting made it difficult for physicians to get the data they needed to close care gaps and improve health outcomes, as well as for the health system to get reimbursed in value-based care arrangements. St. Joseph’s Health turned to Oracle Health Quality Management, Oracle Health Care Coordination Intelligence, and Oracle Health AI Data Platform to unify its patient records, empower clinicians to close care gaps more efficiently, and lower hospital readmissions for key conditions.
St. Joseph’s Health was an Oracle Health Foundation EHR customer before implementing Oracle Health Quality Management, Oracle Health Care Coordination Intelligence, and Oracle Health AI Data Platform to break down patient data silos, automate workflows, and connect processes. The health system values Oracle Health’s robust cloud platforms, which consolidate data from across EHRs, payers, and community sources to create a comprehensive patient record. St. Joseph’s also values Oracle’s commitment to system interoperability, its AI-powered agents for clinical documentation, and the fact that it brings advanced analytics into day-to-day patient health management. Beth Kushner, the health system’s chief medical information officer, also sees Oracle’s involvement in so many different industries as a plus. “You can take something from credit card processing, and you’ll see it in the rev cycle component,” Kushner says. “Oracle is modifying their business solutions to be more healthcare-centric, but they’re based on solutions that have been in the works for decades.”
By implementing Oracle Health Foundation EHR, followed by the Oracle Health analytics, care coordination, and AI solutions, St. Joseph’s Health has been able to make improvements to many parts of its operations. Clinicians can now access records that provide a single longitudinal view of each patient—across both the St. Joseph’s system and third-party care and claims systems—giving clinicians comprehensive patient summaries, which are critical to proactive care management.
Teams are able to identify and close care gaps quickly and intervene early to address minor concerns before they require costly treatments and help patients better manage chronic conditions. For example, St. Joseph’s Health is using advanced data analytics capabilities in its Oracle Health solutions to identify patients who are repeatedly readmitted for congestive heart failure. In response, the organization has implemented a targeted care program, which has resulted in a sustained reduction in readmissions and a readmission rate that has remained below the market average for three consecutive years.
St. Joseph’s Health has also automated and standardized both inpatient and outpatient care management processes, helping staff concentrate on high-impact interventions. AI capabilities embedded in the Oracle Health solutions provide physicians with relevant data in the appropriate context. Kushner has reassured clinicians concerned about ease of use. “I told one doctor: You did organic chemistry. You’re going to figure this out,” she says. “It’s one big button. You’re going to be fine.”
