Northern Health is one of Victoria’s largest public healthcare networks, serving Melbourne’s rapidly growing northern suburbs with a comprehensive range of clinical services. To support its Hospital in the Home (HITH) program, the provider needed to replace manual coordination across phone calls, spreadsheets, and whiteboards with a scalable, automated platform. By deploying Oracle Service and Oracle Health, Northern Health built a unified foundation for community-based care. This platform provides clinicians, coordinators, and patients with real-time visibility into home visits, enabling seamless, patient-centered care delivered in the comfort of the patient's home.
Why Northern Health chose Oracle
Northern Health’s HITH program delivers acute care directly to patients in their homes. Previously, the team managed these distributed care workflows through manual processes, phone calls, spreadsheets, and whiteboards, making it difficult for clinicians and coordinators to coordinate referrals, triage patients, optimize routing, and monitor activity.
Rather than building a custom solution in-house, Northern Health selected Oracle Fusion Service and Oracle Fusion Field Service as a scalable, configurable, and unified workflow foundation. Oracle Fusion Service streamlines patient referrals, triage, care plans, and communication, while Oracle Fusion Field Service drives AI-powered clinician scheduling, routing, dispatch, and mobile workforce management. Seamlessly connected with electronic health records from Oracle Health, the solution provides field visibility.
Results
Northern Health has modernized how its HITH team coordinates care, giving teams a more connected way to manage patients, clinicians, and home visits. The implementation gives coordinators and clinicians end-to-end visibility into distributed HITH operations, helping to reduce manual work and support more efficient scheduling, communication, and home visit coordination.
In practice, the platform brings referral intake, triage, clinician scheduling, booking, routing, and workflow visibility into a single operating model. AI-powered scheduling and route optimization help teams allocate clinicians more effectively and reduce reliance on manual planning. Mobile-enabled workflows give clinicians better access to information in the field, while centralized visibility helps coordinators see the status of home visits and identify issues more quickly.
Oracle Field Service Mobile App CarPlay enables clinicians to safely and conveniently access their activities, navigation, and maps directly from the vehicle dashboard, helping them manage their schedule more efficiently while on the road.
Northern Health can now deliver a better patient experience through improved communication and transparency, including estimated arrival times for care team visits.
The solution also establishes a scalable foundation for additional community-based services, with Northern Health now evaluating the viability of expanding it to cover other services such as maternity at home.

Oracle Consulting implemented the project over approximately six months using a single-release, big-bang deployment for the HITH team. The engagement included solution design and build, healthcare integrations, familiarization sessions, solution playback workshops, end-user training, a structured go-live approach, and post-go-live hyper care.
