(Updated December 2015)
A small number of power cords from one specific manufacturer may develop a leakage current condition over an extended period of time (typically >4 years). This leakage current is internal to the female plug of the cord. If excessive enough, this leakage current could lead to a trip condition of the rack PDU or external circuit breaker. In a more severe case, there is the possibility of damage due to a thermal event.
These cords shipped with certain systems from January 2004 to March 2015*.
* Updated from July 2011 to March 2015 in June 2015.
The following systems could have been shipped with these power cords:
* Added June 2015
** Added August 2015
Only the following power cords that have the C19 connector as shown are suspected:

Shown below is the label on the suspected power cords. Only power cords with an Oracle Code of 0001205, week 01 year 04 to week 26 year 11 may be affected.
Figure 1: Label Detail
If the label is missing the power cords can be identified by the identifying mark which is located on the female end of the power cord, as shown in the picture below.

If your Sun Rack 900/1000 is configured with 2 vertical rows of up to 6 power strips on either side of the rack you should replace the Rack PDU to Rack Power Strip power cords. If your Sun Rack 900/1000 does not have this configuration you do not need to replace the Rack PDU to Rack Power Strip power cords.
Shown below is a Sun Rack 900/1000 (viewed from the back) configured with 12 power strips on the left side of the rack:

Note: your Sun Rack 900/1000 may have between 2 and 12 power strips configured in any of the locations shown (A0 through A5 and B0 through B5)
For each configured side (as viewed from the back) you should request the corresponding replacement kit:

Note: replacement part numbers and power cord lengths will vary from the original component parts. Replacement kits contain 6 cords per kit: consider the number of kits required based on the number of cords that need to be replaced.
These power cords should be replaced when replacements are received from Oracle. Power cords are customer replaceable items, so if you choose to do the replacement yourself:
These power cords may be used on products other than those with which they originally shipped (see list above). If you believe that cords may have been moved and used elsewhere in your datacenter, inspect those other systems to confirm their power cords are not impacted by this issue. If you find they are indeed affected part numbers, as identified by the Oracle label shown above, proceed with the actions outlined herein to replace and destroy the cords.
The reliability and quality of our products is important to Oracle. Oracle will replace suspected power cords connected to Oracle systems and Oracle-labeled suspected power cords connected to non-Oracle systems. To obtain replacement power cords, or if you have questions, please use My Oracle Support and open a hardware Service Request (SR) using the serial number of one of your SPARC T5-4/SPARC T5-8, M4000/M5000, Netra 6000, SB8000/SB8000P, SB6000/SB6048, ST6540/ST6580/ST6780 products.
Create one hardware SR for all systems at one location, using a serial number from one of those systems. Please select the Problem Type "Power Cord Field Change Order". This Problem Type is only visible when logging the SR using one of the affected system serial numbers. You will be prompted for the following information:
If you have systems that are not on the list of systems these power cords shipped with but do use suspected power cords, use a valid serial number of one of those systems to open the SR and mention "Power Cord Replacement Notice" in your problem description.
If you do not have access to My Oracle Support, please contact Oracle by phone. You can find the list of contact numbers in the Oracle Global Support Contacts Directory.
Upon submission of an SR with complete information, Oracle will arrange to ship the replacement cords to you.
