Eastern Health modernizes in-home care with Oracle Service and Oracle Health
Oracle Fusion Cloud Service and Oracle Fusion Field Service help unify visit planning, AI-powered scheduling, routing, and mobile care operations.

“Oracle helps our teams coordinate in-home visits, plan routes, and manage daily field activity in a more connected way—supporting more efficient and safer care delivery and improved patient satisfaction.”
Making care easier to coordinate beyond hospital walls
Eastern Health is one of Victoria’s largest public health providers, delivering hospital, community, mental health, and specialist care across Melbourne’s eastern region. Through its Care@Home initiatives, including Hospital in the Home, GEM@Home, and Rehab@Home, the organization provides critical hospital-level care directly to patients where they live.
However, rapid growth stretched manual operational models to their limit. Care coordinators relied on multiple spreadsheets, phone calls, and physical whiteboards to assign daily home visits. Clinicians on the road worked from transcribed paper schedules, leaving dispatchers and nurse unit managers with limited real-time visibility into delays, unexpected schedule shifts, or travel conditions.
Eastern Health needed to modernize how in-home care services were coordinated and delivered. The goal was to give care teams a more connected operating model for managing home-based care, enabling them to respond faster and deliver a better patient experience.
Replacing manual processes with connected workflows
Eastern Health partnered with Oracle to build a unified operational foundation for home-based care. Oracle Health brought healthcare context, while Oracle Fusion Cloud Service and Oracle Fusion Field Service provided the capabilities for appointment booking, work orders, clinician scheduling, activities, routing, and field coordination.
With the AI-powered scheduling, dispatch, and route optimization in Oracle Field Service, Eastern Health can better match visits based on the clinician’s skills and availability and the patient’s location and daily care needs. Rather than relying on disconnected tools to orchestrate a day’s work, teams can manage key care coordination activities through a single system.
Oracle Customer Success Services supported the implementation with solution design, onboarding, training, and post-go-live support. The phased approach helped Eastern Health move at a pace that worked for clinical teams. It also gave the health service a practical way to shift from manual scheduling and paper-based field coordination to connected workflows designed for distributed, mobile care teams.
Giving care teams a clearer view of each day
With Oracle Fusion Cloud Service and Oracle Fusion Field Service, Eastern Health can coordinate patient care workflows, schedule clinician visits, and support route planning in one place, thereby replacing spreadsheets, whiteboards, and paper schedules as the backbone of daily field operations.
For care coordinators, that means a clearer view of appointments, clinician availability, home visit activity, and optimized route options. For clinicians, it means better support for daily routes and field work. For patients, it can mean a more responsive care experience as teams coordinate visits with greater speed and consistency.
The shift is especially important in an environment where plans can change quickly on any given day. Teams can use the platform to manage work as it evolves, allowing them to respond more effectively to changes in demand, availability, and travel conditions. Eastern Health reports that it has achieved positive outcomes after implementation, including faster scheduling, improved visibility and activity forecasting, and greater operational efficiency. The scheduling and route-mapping capabilities have also helped teams work more efficiently and safely as they manage home visits.
Building the future of distributed healthcare
As the first health service in Australia to deploy Oracle Health alongside Oracle Fusion Field Service for home-based care, Eastern Health is establishing a scalable foundation for distributed healthcare. The project is about more than increasing capacity. It’s about redesigning how in-home services are planned, coordinated, and delivered—reducing operational complexity, supporting faster responses, and helping clinicians spend less time managing logistics and more time caring for patients where they need it most.
